HEALTH EDUCATION COORDINATOR/COMMUNITY HEALTH MANAGER

The Jessamine County Health Department is accepting applications for a full-time Health
Education Coordinator as classified by the Local Health Personnel merit system to serve as the
Community Health Manager.

General Duties Include: Responsibilities of this position include, but not limited to; manage
Community Health staff that include Health Educators, Worksite Wellness and Regional
Epidemiologist; Coordinate Maternal and Child Health (MCH) program to oversee and work with
assigned staff of chosen MCH packages; participate in assigned Healthy and Safe Communities
Coalition workgroups working with mental health and safety issues in Jessamine County; manage all
JCHD internal and external communication. This will require a strong skill set working with
multiple media outlets such as: social media, press releases, advertisements, radio and TV
interviews, website maintenance, newsletters, etc.; work closely with leadership to lead Community Health staff in Performance Management and Quality Improvement projects; assist leadership with Community Health Assessment/Community Health Improvement Plan (CHA/CHIP) and Public Health Accreditation Board (PHAB) Reaccreditation efforts; work on public health policy development; monitor data that impacts health status for Jessamine County; work closely with leadership on overseeing Community Health budget and resource allocations and provide direction on Community Health programs, operations and expenses; assist with grant writing and grant application process; attend local, state, national meetings for workforce development and networking opportunities.

Minimum Education, Training and Experience Requirements:

EDUCATION: Graduate of a college or university with a bachelor’s degree.

EXPERIENCE: Must have five (5) years of health related or education experience.

SUBSTITUTION FOR EDUCATION: Master’s degree or bachelor’s degree in health or human services fields will substitute for one (1) year experience required. A professional license in the health or human services fields will substitute for an additional year of experience

Starting Salary: $16.89-$21.42 may be adjusted at agency’s discretion based on qualifications

Worksite Wellness Coordinator

The Jessamine County Health Department is accepting application for a full-time Health Educator I, II, or III or a Nutritionist, I, II, or III as classified by the Local Health Personnel merit system to serve as the Worksite Wellness Coordinator (WWC).  This position will report to the Community Health Manager.

General Duties Include:  Responsibilities of this position include, but not limited to; lead, organize, coordinate staff, and evaluate the Go365 wellness program for Kentucky Employee Health Insurance Program members in Jessamine and surrounding counties to help members achieve a healthier lifestyle through biometric screenings, and providing health education and resources; organize, coordinate, and evaluate the JCHD Diabetes Prevention Program and Diabetes Self-Management Program; WWC will provide wellness programs to the local business industry to help produce a healthier workforce by planning, design, implementation, delivery and evaluation of worksite wellness programs (including but not limited to: tobacco cessation, nutrition education programs, health screenings/vaccinations, health fairs, awareness events and classes, health communications, health coaching, lifestyle interventions, health education, injury prevention, stress management, and high risk management programming); WWC will work with employers to review and/or develop internal agency policies to create/enhance a healthier workplace environment; WWC will work closely with the Maternal and Child Health Coordinator (MCH) to lead Healthy People Healthy Communities MCH package; participate in assigned Healthy and Safe Communities Coalition workgroup working with chronic disease issues in Jessamine County; organize and coordinate staff with off-site flu shot clinics for worksites in Jessamine County; lead JCHD worksite wellness programing for staff; WWC will also be responsible for general health education duties requested from the community such as hygiene education, handwashing demonstrations, health fairs, etc.…This person must have the ability to travel to local, state, and some national worksite wellness meetings and/or conferences.

Minimum Education, Training and Experience Requirements:

Education:
Health Educator I, II, or III:  minimum qualifications: Graduate of a college or university with a bachelor’s degree and one (1) year of health related or education experience.

Starting Salary:  $12.15-$19.96 (contingent based upon qualifications of merit classification)

Substitution for Education: Master’s degree or bachelor’s degree in health or human services fields will substitute for one (1) year experience required. A professional license in the health or human services fields will substitute for an additional year of experience.

Nutritionist I, II, or III:  minimum education requirements:  Bachelor’s Degree with a major in community nutrition, public health nutrition, foods and nutrition or dietetics OR Bachelor’s Degree in home economics/home economics education, food and consumer science or family and consumer science with eighteen (18) semester hours in foods and nutrition or dietetics.

Starting Salary: $12.15- $18.63 (contingent based upon qualifications of merit classification)

Substitution of Education:  Three (3) years of experience as a Nutritionist or three (3) years providing nutrition and health-related counseling and education and successful demonstration of competencies for the Nutritionist II.
